Our clients are among the most vulnerable citizens in Hamilton county, dealing with co-occurring disorders, homelessness, and incarceration. Join us in developing equity in Hamilton County by providing trauma-informed advocacy, care, and support for our clients.
Sign-on Bonus
$4,000
- $2,000 at 90 days of employment
- $2,000 at 6 months of employment
On-Call Pay
All ACT employees are expected to rotate on-call duties. Employees will be paid for being available to respond to crisis calls.
- $16.66 per weekday
- $33.33 per holiday and weekend
Additionally, any employee who must respond in person or beyond the length of a short phone call, will clock in for their normal hourly rate after conference with team lead.

Position Summary
The Registered Nurse is responsible for overseeing all aspects of medical care for those individuals RHD provides services.
Essential Duties and Functions
- Act as part of a multidisciplinary team designed to provide services from a holistic and values based perspective
- Manage the ordering/delivery/storage/administration of medications for the persons served in accordance with the specific program/service
- Provide education to both persons served and staff regarding common health concerns. Provide training to staff as required/requested in order to best meet the needs of persons served.
- Assess and respond to new/potential/ongoing health issues in persons served.
- Complete documentation as required by RHD, funder/licensing/accreditation bodies.
- Conduct screening and assessment in accordance with the use of validated screening tools to screen and assess for depression, trauma, substance use, and risk/needs associated with criminal risk.
- Document treatment contacts using collaborative documentation.
- Demonstrate the ability to provide treatment that is recovery-oriented, supports evidence-based practice and can clearly establish the status of the client in treatment.
- Function as a liaison for recipients in securing medical care, and other community supports that promote integrated physical and mental health wellness.
- Coordinate medical care in coordination with the prescriber and other medical providers.
- Administer IM's per physician orders.
- Work with consumer & team to develop strategies to improve medication adherence.
- Monitor vitals signs and side effects of medication and reports findings to team,
- Work with team to ensure all consumers have a primary care provider, receive annual physicals and dental examination, and maintain prescription schedule.
- Meet monthly required levels of treatment contacts.
- Provide 24-hour emergency and crisis intervention response on rotating, on-call basis.
- Complete assessments, progress notes, recovery plans and significant data in required time periods.
- Assess medical care needs and other services to support wellness.
- Recommend health objectives for consumer's recovery plans.
- Provide culturally competent services that are individualized to client needs and reflect the integration of their race, color, religious creed, disability, handicap, ancestry, sexual orientation, gender, gender expression, national origin, age and sex.
- Perform other duties as required by Team Leader/Program Director.
Qualifications
- A graduate of a school of professional Nursing (AD, hospital, or BSN); BSN preferred;
- Current license to practice in the state of Tennessee

- Medium: exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects.
